Transform Your Health Today With HIS ConnectCare™!

Care That Stays Close

HIS Collaborative is a Washington DC-based whole person care nonprofit led by Rev. Madeline "Pastor MAD" Long. We connect medical, behavioral, social, and legal support through neighborhood Hybrid Care Sites and the HIS ConnectCare™ platform, bringing coordinated, trusted care and remote monitoring tools directly into local communities.

Leadership Shaped by Experience

From early work training DC residents for stable jobs, to building digital cable systems, to walking with women through breast cancer and families through crisis, Pastor MAD has seen how health, income, faith, and justice are tied together. That lived wisdom is the heartbeat of HIS Collaborative's whole person care.

Centered on Human Dignity

Our story matters because it centers people, not programs. Guided by Pastor MAD's lived experience, we focus on families who feel unseen, link them to primary and behavioral care, respond to housing and legal emergencies, and use secure digital tools so support remains close, consistent, and compassionate.

Our Mission And Vision

We work to build trust, reduce health disparities, and transform lives by uniting medical care, mental health, housing, nutrition, legal support, and community strength into one coordinated whole-person care experience.

Rev. Madeline “Pastor MAD” Long is a dynamic leader whose life’s work spans workforce development, business innovation, public health advocacy, and ministry. Her early career training DC residents for stable employment, followed by her success co‑owning a cable construction company featured in Black Enterprise, reflects her commitment to building opportunity and strengthening communities. Her leadership later expanded into public health, where she reached thousands of women with lifesaving screenings and support.
